It’s no secret that patients are struggling to access care. To combat this, health systems can adopt a hybrid care model, or a strategic combination of in-person and virtual visits. This model operates under the premise that there’s no “one-size-fits-all” solution and gives providers the discretion to utilize all visit tools at their disposal to tailor care specifically to a patient’s needs. For example, a patient with a chronic condition can be remotely monitored at a more regular cadence but easily schedule an in-person visit if their condition worsens.
